Solution for missing Toastmaster magazine

As agreed by WHQ, the solution for the delivery of Toastmaster magazine is:
經世界總會同意，宣佈解決Toastmaster 雜誌寄送問題之方案如下:


for the back issues (missing) magazine:
過期遺失的期刋問題：


1. WHQ will resend the missed back issues by bulk shipping to individual club. Fill the application form for bulk delivery, as attached file.
世界總將依大量信件方式將未收到的過期雜誌寄到各分會。依附件表格內容填寫完全。


2. The deadline of Club is Aug. 21, 2009.
分會調查及填寫表格之最後期限為八月二十一日前。



Please help me to fill the attached file by providing your name, ID, the months you didn't receive your magazine. I will reply to Area G1 governor, Jewel Hsu, before August 21st. Thanks so much.
